Qualifying Criteria:
A. Qualifying Criteria for Collaborating Agency
|
|
|
Gifts For Seniors is designed to collaborate with agencies /
organizations / places of faith (herein referred to as
‘agency/ies’) that meet this set of criteria: |
|
|
.:: |
provides face-to-face service(s) to isolated senior citizens who
meet the criteria listed in the section below, |
|
|
.:: |
have no or very limited winter holiday gift donations
to provide to isolated senior citizens,
is able to provide authorized staff and/or volunteers
to: |
|
|
|
.:: |
complete and submit the:
application materials and results through written reporting
process,
comply with the Accountability and Responsibility Agreement,
attend the
January 10, 2008 meeting of all Collaborating
Agencies, |
|
|
|
.:: |
select and pickup donated gifts by appointment at the
Internal Operations Center in Roseville in December
and/or January on Tuesdays, Wednesdays and/or Fridays,
mornings and/or afternoons, |
|
|
|
.:: |
match donated gifts to the isolated seniors’ needs, and |
|
|
|
.:: |
arrange for the gifts to be personally delivered as a surprise
to the isolated seniors from December through March
with an attached Gifts For Seniors gift tag. |
|
|
|
B. Qualifying Criteria for isolated senior citizens |
|
|
|
Gifts For Seniors is designed to serve isolated senior citizens,
regardless of income, who have little or no contact
with family or friends. Gifts donated to Gifts For
Seniors are given to seniors through the Collaborating
Agency. Isolated senior citizens receiving a gift
meet this set of criteria: |
|
|
|
.:: |
Are 60 years of age and better, |
|
|
.:: |
Are isolated or forgotten - meaning the seniors are not expected
to receive any other winter holiday gifts -, have
little or no positive contact with family or friends,
and are among the most isolated seniors the agency
serves, |
|
|
.:: |
are clients/residents/patients/participants/parishioners of the
collaborating agency, and |
|
|
.:: |
reside in one of these 12 metropolitan counties of the Twin
Cities,
Minnesota:
Anoka, Blue Earth, Carver, Chisago, Dakota, Hennepin,
Nicollet, Ramsey, Rice, Scott, Washington, Wright. |

|
|
C. If your agency meets the two sets of criteria above:

D. Use this checklist of application materials to complete
and submit:
1.
A cover letter on your agency’s letterhead that provides:
a.
Your agency’s mission statement,
b.
The service(s) your agency provides to isolated senior
citizens,
c.
The days and times of the week that work best for staff
and/or volunteers to pick up *gifts at the Internal
Operations Center in Roseville – choices: Tuesday’s mornings
and/or afternoons, Wednesday’s mornings and/or afternoons,
Friday’s mornings and/or afternoons. *Gifts include:
unwrapped gifts, SunShine Club gifts, gift
cards/certificates and prepaid items such as postage stamps,
transit cards.
d.
Staple your business card to the cover letter.

Note:
The SunShine Club referral form and briefing will be
provided to agencies that wish to collaborate in this way
with Gifts For Seniors.
